DOCUMENTS NEEDED TO PROCESS A PASSPORT
- Passport Application. (may be obtained at travel.state.gov or in our office) Complete using BLACK ink in full (both pages for minors and adults) prior to your appointment time.
- Certified copy of birth certificate (must have a raised seal - hospital certificates are not acceptable). Birth certificates are mailed in with your application but returned to you in a separate envelope from your passport. Birth certificates can be obtained from your local health department. Birth certificate should be long form certificate with parental information. How to obtain an out of state birth certificate. If this is your first adult passport you are required to bring in a certified copy of your birth certificate and cannot use a prior passport as citizenship evidence.
- Passport photo. We do not take passport photos in our office. These can be made at any location that handles passport photos. Example: CVS Pharmacy; Walgreen's; Wal-Mart photo shops, .
- Two checks. You may pay using personal/business checks, cashier’s checks or money orders. We cannot process your application using a debit and/or credit card. For an adult passport you will need one check made payable to the U.S. State Dept. for $130.00 and one payable to Mary Harris for $45.10 (this is $35.00 execution fee plus $10.10 for priority mail). Should you have to expedite the process, the check to U.S. State Dept. should be for $211.36, and the check made payable to Mary Harris is $66.40 ($35.00 execution fee and $31.40 priority express mail). We are required to send them in a manner that can be tracked via USPS. USPS is strongly encouraging passports be sent via Overnight Express due to mail delays. (mail rates effective 1-19-2025)
- Valid Driver's License or photo ID. In the event your driver’s license has expired and you have a temporary license we will need copies of both. If you Driver's License or photo ID is from another state, you will need to provide a second photo ID.
It is currently taking from 4 - 6 weeks for the National Passport Agency to process your application. Expedited service is taking approximately 2 - 3 weeks. Please contact our office if you are traveling in less than 2 weeks.
Once you have acquired all of the above documents and completed the passport application, please call our office to make an appointment at (205) 670-5270. As of February 1, 2004, all passport applicants must appear in person. (Including minors) Also, as of 2-1-2008, any minor under the age of 16 requires the consent of both parents and both parents must be present or you must have the statement of consent form completed and notarized with a copy of the identifying information for the other parent.
The above information is for application for a first-time adult passport. If your passport was issued less than 15 years ago and you were an adult when you received your passport you may renew your passport yourself without appearing in our office by completing the renewal application at travel.state.gov.
